WITH THE PRACTICE?
You will be asked
to complete a health questionnaire. Adults will be given
an appointment with the practice nurse who will ask about your medical history
and current medication etc. This will assist in assessing your health needs prior
to receiving your records from your previous doctor. A brief health check will
be performed so please bring a specimen of urine with you. In the case of children
under six you will be asked for details of their childhood immunisations. This
will ensure that your child is included in the computerised recall system operated
within this area.
